• Perio maintenance cleanings (3 to 4 per year)
• Doctor exams (1-2 per year)

• X-rays (as needed)
• One emergency exam per year (if needed).

*For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease.

Child Membership (12 & Younger) – $30/Month

Getting Started

This plan is effective immediately upon purchase. Payment must be made by credit or debit card every month; we accept all major cards, including Visa, Mastercard, Discover, and American Express. To sign up, we will need the following:

• Email address (to send receipts)
• Credit or debit card
• Your full name, address, phone number, & date of birth

Begin saving today by calling and scheduling your appointment (be sure to mention that you’re a member)! On your appointment date, we will check to ensure your payment is current before proceeding with your cleaning, exam, preventative treatment, or another service!
